SUMMARY OF EMPLOYMENT CONDITIONS

Based upon state and federal regulations, the following employment conditions are required once a job offer has been accepted by a qualified applicant.

Employment will be offered pending...

1. MANTOUX TEST (TB test) passed successfully. (This may be administered by our staff at no cost to the applicant, or by a physician at the applicant's expense)

2. SUBSTANCE ABUSE SCREEN passed successfully at Cardinal Center's expense.

Note: if the above screen returns positive the applicant will not be eligible to reapply for 1 year.

3. CRIMINAL HISTORY REPORTS AND MOTOR VEHICLE CHECK (includes States, Counties, CNA and Sex Offender) with acceptable results.

4. PROOF OF DEGREES, HS DIPLOMA OR GED

5. HEPATITIS B INOCULATION OR WAIVER SIGNED for all positions where it is reasonably anticipated the employee will be exposed to Hepatitis - this will be at Cardinal Center's expense.

Note: A DOT physical examination, substance abuse screen, and a Mantoux test passed successfully, at the company's authorization and expense, will be required for all positions requiring a commercial driver's license.
